When does a bone marrow transplant become necessary?

What are bone marrow transplants and its types?

There are three basic types of transplants, allogeneic, autologous, and umbilical cord blood transplant.

Donor and Recipient are two separate individuals and the transplant is done using the stem cells of the donor.

  • Matched Related: Where the donor is an HLA-matched relative usually a sibling.
  • Matched Unrelated: Where the donor is not a relative of the patient and is usually found from one of the various national or international registries.
  • Partially Matched Related: Where the donor is from a patient’s family but partially matched (haploidentical).

Donor and Recipient are the same individuals, where the transplant is done using the patient’s own bone marrow. The procedure involves giving high-dose chemotherapy to patients in order to remove the primary disease. 

Thereafter, an autologous transplant is conducted to rescue damaged bone marrow. This type of transplant has minimal complications and is preferred for diseases like multiple myeloma/lymphoma.

This is a type of allogeneic transplant. Right after birth, stem cells are extracted from a newborn baby’s umbilical cord. The stem cells are frozen and kept until a transplant is required.

Perfect matching isn’t required because umbilical cord blood cells are so immature.

Blood counts take much longer to recover due to the lower number of stem cells.

How are a donor and recipient matched?

HLA (human leukocyte antigens) matching is used to match patients and donors for blood or marrow transplants. 

HLA are proteins or markers on most cells in your body. Your immune system uses HLA to see which cells belong in your body and which do not.

If two people share the same HLA type, they are considered a ‘match’. It’s much more complicated than blood typing.
